This course is open to all Cancer Care Co-ordinators, Social Prescribers, Care Navigators based in Primary Care, within Kent & Medway, who support patients living with a cancer diagnosis.
Aim of the events:
• To gain an awareness of cancer prevention, risk factors, screening & its uptake, 2ww referrals, safety netting, diagnosis & treatment
• To empower cancer care co-ordinators, who are involved in 3-month cancer care
reviews and ongoing support to the patient through their cancer journey, to have the knowledge and skills to complete a review & signpost to appropriate services and care providers
• Gain knowledge of the immediate and long-term effects of cancer
• To obtain the knowledge and tools to be able to signpost patients according to
physical, emotional and practical support
• Personalised Care Planning
• Health inequalities – how we can support our local communities
